Participation Waiver

Acknowledgment of Risk:

I, the undersigned parent or legal guardian of the minor named above (the "Participant"), acknowledge that participation in the Little Lions Fitness program involves physical activities that could result in injury. I understand that fitness activities may involve risks such as but not limited to: muscle strains, sprains, broken bones, heart-related conditions, and other physical injuries. I realize that every safety precaution is enforced and that injuries may still occur. I also understand that participation may result in possible exposure to illnesses and infectious diseases such as, but not limited to, MRSA, influenza and Covid-19. I knowingly and freely assume all risks, both known and unknown, and assume full responsibility for the Participant’s exposure.

By signing this waiver, I acknowledge that I have been informed of these risks and voluntarily agree to allow the minor to participate in the Little Lions Fitness program. I understand that it is my responsibility to assess whether the program is suitable for the Participant's physical condition and capabilities. 

Release of Liability:

In consideration of the Participant being allowed to engage in the Little Lions Fitness program, I, on behalf of myself, the Participant, and our heirs, assigns, and legal representatives, do hereby release, waive, discharge, and covenant not to sue and forever release the Little Lions Fitness program, its employees, agents, volunteers, staff and any associated parties (hereinafter referred to as the "Released Parties") from any and all liability for any injury, loss, or damage, including death, that may occur as a result of the Participant’s involvement under instruction, and supervision in the Little Lions Fitness program, whether caused by negligence or otherwise.

Medical Consent:

I consent to any necessary medical treatment or emergency medical care that may be administered to the Participant in the event of an injury or emergency situation during their participation in the Little Lions Fitness program. I agree to bear the cost of any medical treatment provided to the Participant.

Parent/Guardian Responsibility:

I, the undersigned parent/guardian, agree to monitor the Participant's condition and well-being during the Little Lions Fitness program and acknowledge that I will immediately notify the program organizers of any health concerns, conditions, or changes in the Participant’s physical abilities that could impact their participation.

Indemnification:

I agree to indemnify and hold harmless the Released Parties from any claims, demands, or lawsuits arising from any injury, damage, or loss resulting from the Participant's actions or participation in the Little Lions Fitness program, including attorney fees and court costs.

Photo and Video Consent: (if you deny permission, please do not initial)

I grant permission for photos, video recordings, or other media to be taken during the Little Lions Fitness program in which the Participant may appear. These images or recordings may be used for promotional or educational purposes, including online and print materials, without compensation or further consent.

Illness:

The Participant must be fever free, without aid of medication, for a minimum of 24 hours before participating in the class. If the Participant has been sick in the past 24 hours, please do not send the Participant to class. If the Participant has had a fever, yellow or green mucus from the nose, vomited or had diarrhea in the past 24 hours they cannot attend the class. If the Participant appears to be aggressively sick during a Little Lions Fitness class, the program director will acknowledge the Participant return back to the classroom. 

Refunds/Credits:

I understand that once payment is submitted for the Participant, I will NOT be refunded or receive credits. I understand that Little Lions Fitness does not give refunds or credits for programs, missed classes due to illness or any other absence, vacation, holidays, weather related or any other reason for school cancellations/closures. 

I understand that if Little Lions Fitness cancels a class due to an internal emergency situation or conflict; the school will be notified ahead of time, parents will receive a notification from Calvary Kid Care at Harvest Fields through TadPoles, and/or an email will be sent by Little Lions Fitness. A make-up class will be added to the end of the program for this cancellation. 

Release of Responsibility: 

I agree to release my child, the minor Participant, from their preschool classroom to Little Lions Fitness personnel during the program's class times.

Acknowledgment and Agreement:

I have read and fully understand this waiver and release of liability. By signing this document, I voluntarily agree to all terms and conditions outlined above and give my consent for the minor to participate in the Little Lions Fitness program
